Visitors to Canada Medical Expense Claims
If you are a visitor to Canada and you suffered a sickness or injury during your covered trip that required medical treatment and you incurred out-of-pocket expenses.
Option 1: Download, fill out & return claim documentation
Download and complete the claim form(s), attach all required documentation below and mail the required documentation along with your original signed, claim form to the address:

Doctor, Hospital and/or Prescriptions:

Original receipts for any out of pocket medical expenses. Also, please submit documentation to support the diagnosis and treatment of the sickness or injury. |

Explanation of Benefits （If You Have） |

Primary Health Insurance Carrier:

Submit the paperwork you received back from your primary health care provider, showing what they paid and what was not covered.

Option 2: Call us to report a claim
You can initiate a claim by contacting our underwriter's Client Services Department who will help you with your claims. The Client Services representative will record the initial information concerning your claim and send you the documents required along with the information on completing and submitting your claim.
